Applications and Forms

The admittance process is competitive. The application and forms provided here are to be completed by each applicant. Completion of the admission requirements does not guarantee selection into the Dental Hygiene Program. Due to accreditation requirements, a limited number of positions are available for each entering class. Because of the limited class size, some fully qualified applicants cannot be accepted. All applicants MUST meet with a Rio Salado program advisor to review the applicant file and current selection criteria.
